Dear Sir/Madam:
Please be advised that I was at the ER on May 1/14 with chest pain, I was seen by a doctor give asparin and released and sent home with a requisition for further tests.
One week later and I am not hearing from any one about an appointment for further testing. I then called the doctor’s office circled on the requisition, and was told that they do not have anything for me. When I look carefully at the requisition at the top it was not mine, it belongs to someone else.

How do these mistakes happen, and why, this is crucial and can cause serious problems and/or death.

Let me know how this will be handled.
